1. Have you tested positive for COVID-19 in the last fourteen days?
The following are symptoms of COVID-19: 1. Fever or Chills 2. Cough 3. Sore throat 4. Shortness of breath or difficulty
breathing 5. New loss of taste or smell 6. Unusual headache 7. Unusual muscle or body aches 8. Congestion or runny
nose 9. Fatigue 10. Nausea or Vomiting 11. Diarrhea
2. Have you had any of the above symptoms in the last ten days, whether new or different from your usual health
3. In the last fourteen days, have you been less than six feet of someone with any of the above symptoms, or
someone with a diagnosis of COVID-19 for more than fifteen minutes?
Please do not book a ride if you have answered "YES" to any booking questions. You should refrain from using public
transit until 14 days have passed from onset of symptoms. As a reminder, per the Governor's Executive Order, face
coverings are required when riding public transportation. Thank you and stay safe.
